Karyn Kusama to direct Mouthwashing film at Sony, Scott Derrickson producing

Sony Pictures has the Mouthwashing film rights. Karyn Kusama directs, Manfredi and Hay write, and Scott Derrickson produces, per The Hollywood Reporter.

CONFIRMED

Sony Pictures has picked up the feature film rights to Mouthwashing, the cult horror game from developer Wrong Organ, according to The Hollywood Reporter as relayed by Bloody Disgusting and Fangoria. Karyn Kusama, the director of Jennifer’s Body and The Invitation, is attached to direct, with Matt Manfredi and Phil Hay writing the screenplay. Scott Derrickson is on board too, but as a producer, not a director.

What we know

Bloody Disgusting reports, citing The Hollywood Reporter, that Sony Pictures acquired the film rights from Wrong Organ. Fangoria, also citing THR, adds that the deal was made with both Wrong Organ and publisher Critical Reflex.

The creative team is the exciting part. Kusama directs, per THR. Manfredi and Hay handle the script, according to Bloody Disgusting.

On the producing side, Bloody Disgusting and Fangoria both report that Derrickson is attached to produce, with C. Robert Cargill and Vince Cheng producing for Crooked Highway.

That clears up some confusion. Dread Central’s headline credits the film to “Karyn Kusama and Scott Derrickson,” which reads like a co-directing gig. The body of that same article states plainly that Derrickson is not directing. Every report we checked agrees: Kusama directs, Derrickson produces.

The game itself launched Oct 29, 2026 on PS5, PS4, Switch, Xbox Series X|S and PC via the Microsoft Store, and joined Game Pass. So the film announcement landed the same day the console versions arrived. Bloody Disgusting had already named Mouthwashing one of its Best Video Games of 2024.

What is rumored

This one has a short paper trail. Cosmic Book News says it reported on Oct 16, 2026, via scooper Daniel Richtman, that a Mouthwashing movie was in development at Sony’s Screen Gems. Richtman is a single-source social-media scooper with a mixed track record, and Cosmic Book News is a secondary outlet relaying his claim.

Dread Central also says it reported ten days earlier that an adaptation was in the works. We could not verify how Dread Central sourced that earlier piece.

The trade reports of Oct 29 bear out the broad claim that a film was in development. The specific Screen Gems detail is a different matter. The approved trade coverage we saw names Sony Pictures, so treat Screen Gems as unconfirmed for now.

What we don’t know yet

Plenty. We found no Sony Pictures press release and no statement from Wrong Organ or Critical Reflex, so everything here traces back to the trade reports.

No cast has been reported. No release date has been reported. Nobody involved has said anything about how the script will approach the game’s story, its structure or its tone. If you see casting rumors or plot details in the coming weeks, check where they come from before you get attached.

What it means

For horror fans, this is about as reassuring a lineup as a game adaptation gets. Kusama made The Invitation, a slow-tightening dinner party nightmare that runs almost entirely on dread and unreliable trust between people stuck in one place. She also made Jennifer’s Body, which took years to get the respect it deserved. Both films are about people turning on each other, and that sensibility feels like a natural fit for a story as twisted and claustrophobic as Mouthwashing’s.

Pairing her with Manfredi and Hay, her credited writers here, suggests a team that already knows how to work together. Adding Derrickson and Cargill as producers puts serious genre experience behind the project.

What we can’t say is how faithful the film will be, or how it will handle the game’s darker turns. That depends on a script nobody has described publicly. For now, the confirmed facts are simple: Sony has the rights, Kusama is directing, and Derrickson is producing. Everything else is waiting on the next official word.
